3 Levels of Service

General Information and Guidance (GIG):

Providing client with helpful information to help client advocate effectively with nursing home staff.  This may include, but is not limited to, reviewing compliance with standards of care, federal & state regulations, operating procedures and strategies to hold nursing home staff accountable to their obligations and responsibilities.  Advocate will provide information, as necessary & appropriate, to assist the client with addressing problems or concerns with facility staff.  Such information is based on a variety of credible sources and Advocate’s extensive professional experience to resolve basic issues, problems and/or concerns related to the nursing home’s obligation to provide care and services to Client’s family member;

Care Assessment and Resident Evaluation (CARE):

Providing an assessment of the nursing home’s performance relative to federal and state regulations requiring the care and services provided to the Client’s relative.  This may include but is not limited to a comprehensive  review of the resident’s medical record, incident/accident reports, an onsite observation and interview with resident or family,  Based on the comprehensive assessment of the resident, the nursing home must ensure that residents receive treatment and care in accordance with professional standards of practice, the comprehensive person-centered care plan and the resident’s choices, to meet each resident’s physical, mental and psycho-social needs;

Advocate on Behalf of Client (ABC):

Provide advocate services on behalf of the Client with nursing home staff.  This may include but is not limited to reviewing the resident’s medical record, discussing issues with nursing home staff, assessing the accuracy, completeness and/or appropriateness of the resident’s Comprehensive Care Plan, attending Care Plan meetings, visiting resident to observe care & services, requesting copies of medical records and other interventions that constitute effective advocacy on behalf of the Client and Client’s relative.
